BIFFA has launched an investigation into the damage caused by an Island Waste truck at Frank James Hospital in East Cowes.
The impact of the vehicle on an Edwardian pillar caused the top of the masonry to break off and smash on the ground.
It came just days after an Island Waste lorry overturned on the dual carriageway, Medina Way.
Biffa Group's head of safety, health and quality, Matthew Humphreys, said: "We take health and safety very seriously and both incidents are being fully investigated to determine the causes. We are unable to comment on these incidents any further until investigations are complete."
Tanja Rebel, from East Cowes, said: "I went to look and was saddened to see the damage. The wall is listed just like the hospital itself, so I hope it will be repaired as soon as possible and I hope it will be repaired well.
"Let's hope this leads to the council in general taking more care of its heritage."
